URGENT – Have your say on Pondtail Surgery or risk it closing.

This survey aims to find out the views of patients registered with Pond Tail Surgery on how local GP services should be provided to them in the future. This follows the sad passing of Dr Khan-Gilbert earlier this year. Dr Shazia Chaudhary and her team are providing care in the short term.

To develop a longer term solution we want to understand what your priorities are for future service provision and what matters most to you. In sharing your views you will be helping us to inform the decision that the CCG needs to take next year regarding how GP services will be provided for local people.

These options could include the following:

1. List Dispersal. This means that Pond Tail Surgery would close, and we will support patients to register with alternative GP practices.

2. Procurement. This means that the CCG would put the Pond Tail Surgery contract out to bidders and if other providers come forward, one of them could take the surgery on and continue to run the service if a suitable site can be identified. The procurement process would be open to local and national providers.

The survey should only take a few minutes to complete. The closing date of the survey is Monday 4 January 2021.
